It's a billboard advertisement that has a picture of the doctor on it, and his eyes are huge and piercing. There's tons of ysis online written about this topic...as it stands as one of the main and most-apparent symbols used by Fitzgerald. CliffNotes.com probably has a section on it. While the eyes are "the eyes of God", they are not portrayed as loving or caring, but rather judging and surveilling...watching the sins of the residents.
